Does this tell me for certain that a URL is indexed?
It tells you whether the URL can be indexed. Use the Check on Google link or Search Console to see whether it actually is.
Why does a URL show “canonical points to another URL”?
The page names a different URL as the main version, so Google will usually index that one instead.
